Blood sugar control improved as well.  Fasting glucose, insulin levels, and insulin resistance all decreased.  Notably, several inflammation markers linked to heart disease also dropped, including CRP and a compound called TNF-alpha.

Why the benefits showed up most in people who needed them most

Researchers noticed an important pattern buried in the data.  Specifically, adults who started with unfavorable cholesterol, blood pressure, or blood sugar numbers saw the biggest improvements from garlic.  Meanwhile, those starting from healthier baselines saw smaller shifts.
