Popular hair loss drug linked to severe psychiatric harm

Concerns about depression from finasteride emerged as early as 2002, when researchers reported 19 patients developing depression during treatment. They recovered after stopping, and two who agreed to rechallenge relapsed immediately.
Between 2017 and 2023, eight independent studies analyzing adverse event reports and healthcare records from multiple countries all reached the same conclusion: finasteride significantly increases risk for depression, anxiety, and suicidal behavior.
Yet the FDA didn’t acknowledge depression as a side effect until 2011 – nine years after initial reports. Suicidal ideation wasn’t added to the label until 2022, twenty years later. The European Medicines Agency only acknowledged suicide risk in 2025.
The magnitude of harm
Prof. Brezis estimates that over 20 years, hundreds of thousands of men worldwide may have endured finasteride-related depression, while hundreds to thousands may have died by suicide.
By 2011, only 18 suicides had been reported to the FDA – yet based on the 4.6 million people taking finasteride, between 6,440 and 12,880 suicides would be expected. By 2024, only 320 suicides were reported when 19,320 were statistically expected.
Families and doctors often don’t connect a cosmetic hair loss drug to a psychiatric crisis. Patients who complete suicide can’t report. And Merck repeatedly argued that causality couldn’t be established.

Entire sections of the FDA’s 2010 safety review remain redacted as “confidential,” making it impossible to know what regulators knew.
